In the event of a medical emergency, the undersigned Parent(s)/Guardian(s) of the above named participant(s), hereby grants authorization to Propel Swim Academy (PSA), and its representatives, to employ any legally licensed physician or health care facility on behalf of each of the undersigned, and to direct and/or order emergency medical treatment for the above named participant(s) and consent to the immediate administration of life sustaining measures as deemed necessary. Each of the undersigned further agrees that neither PSA nor any of it’s representatives, shall be liable under any circumstances to anyone for exercising the foregoing authority in the event of an emergency. Further, the participant and/or parent/guardian agrees to pay all cost associated with medical care and transportation for the participant.
The undersigned, as the parent or legal guardian of the child(ren) listed on this application in consideration of the request and permission of my son(s)/daughter(s) to participate in the PSA programs, including, but not limited to the Swim Lessons, Birthday Parties, Swim Camps and/or any other swim related activities/events, hereby assume full responsibility for all risks of injury or loss which may result from my son’s/daughter’s participation in this activity and hereby agree to hold harmless, release and forever discharge PSA, it’s third person of any accident, illness, injury, or death of any person and persons, or damage to or loss or destruction of any property arising or resulting from swimming, strenuous exercise, vigorous physical activities. I/we the parent/guardian of the participant(s) hereby agree to indemnify and hold harmless PSA and employees against any liability resulting from any injury that may occur to the participant while participating in, or traveling to and from any of the aforementioned activities. The participant also agrees to indemnify PSA for any damages incurred arising from any claims, demand, action or cause of action by the participant.
I understand, agree, and acknowledge that there are risks inherent in the sport conducted by Propel Swim Academy including, but not limited to scrapes, bruises, cuts & more serious injuries such as paralyzing injuries, brain injuries, and death. These activities may be of a hazardous nature and/or may include activities such as swimming and vigorous physical activities. With the full understanding of the facts, I state, that to the best of my knowledge, my son(s)/daughter(s) listed on this application has no medical, physical, mental or emotional health condition, which would hinder or prevent his/her active participation in PSA programs.
I understand that Photos and Videos (herein and after collectively "Images") are occasionally taken at Propel Swim Academy facility and that any Images taken of my minor child or myself may be used. I here by give PSA my permission to license the Images and to use the Images in any Media for any (except pornographic or defamatory) which may include, among others, advertising, promotion, marketing or packaging for any product or service. I agree that the Images may be combined with other images, text and graphics, and cropped, altered or modified.
I agree that I have no rights to the Images, and all rights to the Images belong to PSA. I acknowledge and agree that I have no right to Consideration or accounting, and that I will make no claim for any reason to PSA.
In the interest of the health of all our swimmers and staff, we have a policy that requires all children under the age of 3 to wear 2 swim diapers (a disposable swim diaper and 1 reusable) during their swim class.
If your child is over the age of 5 (five) they need to use their gender specific restroom. However, we have a family restroom that is non-gender specific located in the changing room area. While it is large enough to change in, we would appreciate it being used only as a family restroom.
We charge an annual Membership fee of $34 per child (max $50 per family), due at registration and recurring on a yearly basis. This will be charged automatically to your card on file. When you first enroll in classes, each student will receive a FREE Propel Swim Academy bag, swim cap, and car magnet! This fee covers the administrative cost of your registration, as well as your welcome bag, unlimited make-up class privileges, and FREE family swim days!
We reserve the right to change our policies and procedures as stated on our website at any time and these changes will be effective immediately upon being posted to the website. As part of your enrollment, you acknowledge this and understand that it is your responsibility to check our website for policy changes periodically.
Students pay for their class spot on a recurring, monthly basis. The primary credit card on file will be billed on the first day of each month. Please note that your account will still be charged on a monthly basis until you withdraw your student by following our Withdrawal procedures (see below). If you register in the middle of the month, your first month’s tuition will be prorated to reflect any missed classes that month. You will be assessed a $10 handling charge on a declined credit or automatic debit transaction. Your account will be automatically billed in this instance. Due to operational costs, no refunds are issued. Credits will only be issued with prior notice for extended medical issues with a doctor’s note.
In order to qualify for a makeup lesson, you must log in to your ‘My Bookings’ page and mark your child absent before the start of their lesson. Makeups will be scheduled only in class spots that are open due to a cancellation of the normal attendee. We offer free family swim twice monthly to allow swimmers who have missed classes to come practice with some of our teachers. Make-Up Lesson availability is not guaranteed. Please see our FAQ’S for further details.
If Propel Swim Academy is closed due to inclement weather or for public holidays (holiday closing schedules can be found in our facility, on our website or by asking any one of our wonderful staff!) we do not issue refunds for the missed classes. In the event that we cancel classes due to inclement weather, we will award a makeup credit for the missed class. We will close for Winter Break in December to perform scheduled maintenance; tuition for December will be prorated accordingly.
You may stop your lessons at any time. By withdrawing, your child(ren) will be removed from their class. A WRITTEN NOTICE is required in order to be removed from the schedule and stop payments. You may email us notifying us your withdrawal request or you may fill out a Withdrawal Form. The form is located under the “Forms” Tab. Verbal or email withdrawal notifications are not valid. There are no refunds. To avoid being charged for the next month you must submit your Withdrawal Form by the 15th of the previous month. (For example: If you wish to stop your lessons for May, you will need to submit your form by April 15th).
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious and is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and possibly in the air. There is limited information regarding COVID-19, with exact methods of spread and contraction unknown. The medical knowledge of COVID-19 is currently evolving, but no known cure, vaccine, or treatment is currently available.
Assumption of Risk: Propel Swim Academy has put in place preventative measures suggested/required by the CDC to reduce the spread of COVID-19; However, Propel Swim Academy cannot guarantee that you or your child(ren) will not become infected with COVID-19.
